If you are logging on for the first time,
use the five digits of your Medical License number as your password.
You will have the opportunity to create a new password on the next
screen. Passwords are case sensitive and cannot be blank.
If your license number consists of only four digits, you will will
need to insert a zero in front to fill the total of five spaces
required. For example, 01234.
